Carolyn Reed, MSW, CSW

Director of Aging Services

I have been at Bear River Aging for 8 years. I am a Master’s Level Certified Social Worker with Clinical expertise. As the Director of Aging Services, I am here to help you sort out the issues that arise as the aging process affects you and your family. With a plethora of experience and education we, at Bear River Aging, can give you the tools you need for a smoother aging experience.

I have been in the social services field for 20 years. I am the long-term care Ombudsman for our Tri-county area, which means I work as an advocate for those living in long-term care facilities. Defending the rights of our most vulnerable populations is something I am passionate about.

While finishing my Social Work degree, my role at Bear River Aging is as the Aging Administrative Associate. I am deeply engaged in advocacy, support, and administrative services for the elderly. My role here at Bear River Aging is to assist with outreach, social media, administrative community connections, senior center liaison, and all around “go to” for the entire Aging Team.

Keirstin Meyer, MS

Fiscal Administrator/ Long-Term Care Ombudsman

I have been with Bear River Aging for 8 years. I have my Master’s in Gerontology which gives me a great insight to the issues the population we represent. I am the Fiscal Administrator and work to keep the funding moving fluidly for all the State and Federal Aging programs we implement.

Melissa Lewis, SSW

Aging Waiver Case Manager

I have been with BRAG for 28 years. I have my SSW and work as the Case Manager for the Aging Waiver program. This program supports seniors to live in their own homes. I also supervise the State Health Insurance programs – SHIP for Cache, Box Elder and Rich Counties.

Shanna Anderson, SSW

Veterans Specialist

As an SSW I have extensive experience working with the aging population. I am the County Veterans Service Officer providing case management, education, referral, and health care benefits connections. My role is to connect veterans to all things related to pension, service connections, and veteran support benefits.

As the Registered Nurse for the Aging Waiver program, I asses the need a client may have for these services. Applying my role as a Social Worker I am the case manager for the Family Caregiver program where family caregivers are assisted with respite, companionship, and support services to give relief to the caregiver.

Intake Specialist/ Caregiver Case Manager/ Alternatives Case Manager

I have been working with seniors for over 15 years. At Bear River Aging, I am the “gatekeeper.”  All inquiries about services and assistance, will be filtered through the Intake process that I oversee. Case management of the Alternatives program is another role I fill to assist in supporting our senior population. This program can assist our seniors with support services to live in their home.

Tammy Hadley 

State Health Insurance Program Specialist

I have worked with seniors for the past 25 years. I work as the Box Elder County State Health Insurance program – SHIP Counselor. My role is to assist clients with Medicare/Medicaid education counseling. I also work with programs to help prevent Medicare fraud and abuse. 

EVENTS

We currently have Caregiver Support Group, Caregiver Academy, Dementia Dialogues, Grandfamilies Support Group and Suicide Support Group events happening in the Cache, Rich and Box Elder Counties.
