OUR PROGRAMS
Age well at home

ALTERNATIVES PROGRAM
Our Alternatives program is geared towards adults who are low income and need assistance with activities of daily living. This program is one of our many programs that are meant to help older adults live at home.

CAREGIVER SUPPORT PROGRAM
Caregivers are faced with a host of new responsibilities, many of which are unfamiliar, intimidating, and can ultimately create caregiver burnout. The Caregiver Support Program provides assistance and support to family caregivers.

MEDICAID AGING WAIVER PROGRAM
The Medicaid program's goal is to help older adults ages 65 years of age and older, that meet nursing home placement, to remain in their homes and not have to go to a facility to get the care that they need.

NEW CHOICES WAIVER PROGRAM
This program is designed for individuals ages 18 or older that are residing in a long term care facility. It gives them the option to move into a community-based setting if they wish to do so.

OMBUDSMAN
Our certified Ombudsman resolves issues surrounding the care and quality of life for people living in nursing homes and assisted living facilities. Our Ombudsman is there to represent the residents' interests. It is the residents' desires and needs that are considered and acted upon.

SENIOR HEALTH INSURANCE PROGRAM (SHIP)
SHIP is a national program that offers one-to-one counseling and assistance to people with Medicare and their families.
