In-home services to support and enable low-income older adults to continue living in their homes.

This program that provides in-home services for seniors that are at risk of nursing home or care facility placement.

Our goal is to improve quality of life while preserving each individual’s independence, dignity, and personal choice through compassionate, client-centered care.

The program connects eligible individuals and families with supportive services designed to meet medical, personal, and daily living needs in the least restrictive environment possible.

What Services are Provided by the Alternatives Program?

Examples of non-medical, in-home services:

  • Case-Management

  • Bathing

  • Dressing

  • Housekeeping/Chores

  • Home Meal Delivery

  • Transportation (depends on elected agency)

  • Medication Devices (e.g. bubble packs etc.)

  • Incontinence Supplies

  • Emergency Response Button

Who Can Benefit From the Alternatives Program?

The Alternatives Program may be appropriate for individuals who:

  • Need assistance with daily activities such as bathing, dressing, grooming, meal preparation, or mobility

  • Are experiencing physical frailty, chronic illness, memory loss, or functional limitations

  • Are at risk of nursing home placement

  • Prefer to remain safely at home with supportive services

  • Need help coordinating care, medical services, or community resources

  • Require short-term or ongoing support to maintain independence

Are you a veteran? You may qualify for additional supportive services

  • To qualify, the care receiver must be at least 18 years of age. While the age range is great, those accepted onto the program generally must be 60+ years of age. 

  •  Must need assistance on two or more personal care activities. Personal care activity examples:

    • Bathing​

    • Showering

    • Toileting

    • Feeding

 

Financial Requirements

  • Single Adults: Less than $1,956 monthly income. Less than $6,000 in assets

  • Married Couple: Less than $2,644 monthly income. Less than $12,000 in assets​​

    • Walking

    • Grooming/Hygiene

    • Getting Dressed

    • Transferring (e.g. from bed to mobility device)

Q: Is there a cost for services?

        A: No. However, eligibility for the program is based on income. If the care receiver(s) make more than the income limit, they may not be eligible for services.
